Please do not send health information
Do not submit symptoms, diagnoses, treatment history, insurance details, medication information, crisis details, or other sensitive health information through this public website form. Clinical information, if needed in the future, should be collected through an appropriate secure clinical system after the practice is properly established.
Third-party form processing
The consultation request form is configured to use FormSubmit, a third-party form-processing service, to forward submissions to the designated XYNOXIA email address. Information you submit is therefore processed by that service as part of delivering the form. Review the provider’s own privacy terms before submitting a request. XYNOXIA has not yet completed a live end-to-end delivery test, so please do not rely on this form for urgent or time-sensitive communication.
